  3. A statement under penalty of perjury that the removal was a mistake or misidentification.
  4. Your name, address, phone number, and email.
  5. A statement consenting to jurisdiction and accepting service of process from the original claimant.

3. Repeat Infringers

We reserve the right to terminate accounts of repeat infringers.
